CASE MANAGEMENT:Case Management is a collaborative process that helps people gain access to services and information with the client being the center of a Case Management Plan.
- Medicaid HIV/AIDS Case Management: As an approved provider by the North Carolina AIDS Care Unit, specialized services for individuals and families infected and affected by HIV/AIDS. Emergency Assistance, Rental Assistance, Transportation, Referrals, Access to Ryan White Title II Funding, Clinical Trails, Drug Assistance Programs, Food Pantry,Group Support, Care Coordination, Benefits Advocacy, Health care information brochures on treatment, care and prevention. HOUSING SERVICES: CSN, Inc has a Housing Counselor certified by The Association of Housing Counselors. We provide pre-homeownership counseling to help new home buyers purchase their first home. We have access to government programs with low interest loans and no down payments. Help with foreclosure prevention, qualifications, and assistance with credit reports. One-on-one counseling and group classes (Homebuyer Education) are offered throughout the year.
Homelessness Prevention limited financial emergency assistance to keep families in their homes by preventing eviction and the cut-off of utilities.
Transitional Housing Program temporary housing for up to twenty fursix (24) months to assist individuals in their search for permament housing. Along with support services: Case Management, transportation, and personal assistance. Rental is based on income. Please call for availability and other requirements.
HOPWA-TBRA- (Tenant Based Rental Assistance Project ) a HUD funded project that works a little like the well known program Section 8. Tenant must be low income at or below 30% (MFI), responsible to pay 30% of their monthly adjustable income toward their rent. The project will be responsible to pay the remainder. IMMIGRANT SERVICES: Assistance with the immigrant population in accessing services, immigration documents, notary services, translation of documents and other personal services. Access to Citizenship Training Classes via partnership with Harnett County Literacy Association. Please call for more details. (Se habla Espanol) 1-866- 509-2863 Classes are available on a bi-monthly basis. Books and material are provided for a small fee of $25.00.
OUTREACH PREVENTION EDUCATION TRAINING (OPET) Outreach services to the community around health and social issues from domestic violence to diabetes. Health fair booths, classroom presentations,workplace presentations. Free literature, Internet Access & Training, condom distributions. Training workshops for consumers as well as for professionals. POL (Popular Opinion Leader) a Community Level intervention
designed to reduce risk for men who have sex with men to identify, enlist, and train key opinion leaders to encourage safer sexual norms and behaviors in their social networks through risk-reduction conversations.
SISTA (Sisters Informing Sisters on Topics of AIDS)

A Group Level, gender and culturally relevant intervention is designed to increase condom use with African-American women. Five peer-lead group sessions are conducted that focus on ethnic and gender pride,( 2 hrs long) HIV knowledge and skills around sexual risk reduction behaviors and decision making.

Monday thru Friday : Hours 3:00-6:00 PM
- Summer Food Service Program 2008 providing a free and nutritious breakfast and lunch for children under 18 years of age in Washington Heights Apartments.
Supported by N.C. Summer Food Service Program. (June-August)
Meal Times: Breakfast 8:30-9:30 AM Lunch: 12:00-2:00 PM.
Please note: Meals must be eaten on site by children !
Training: HIV/AIDS topic, Domestic Violence, Housing, Professional Development, Culture Diversity, Advocacy. Trainings presented are at times with outside Trainers contracted with CSN, Inc. In-house Workshop: HIV/AIDS Stigma & Access to Care.
"PositiveVoices" - a Speakers Bureau composed of individuals living with HIV/AIDS, in substance abuse recovery, victims of domestic violence, cancer survivors, or other social injustices. Available to speak at conferences, community events, churches and workshops. Let us come and speak out at one of your programs! We provide transportation reimbursement and a honorarium.
