Case Management Services
*HIV/AIDS Case Management
Services to individuals and families who are infected and affected by HIV/AIDS. Certified by the North Carolina AIDS Care Unit as a Certified Medicaid HIV Case Managment agency. Designed to improve client access to a wide range of services; to promote continuity of care by coordinating service delivery; to enhance health statusand level offunctioing. Serving Harnett, Johnston, and Sampson Counties. Acess to Ryan White CAREfunding, HOPWA Funding, ADAP, and other resources to assist clients in healthy living. All confidential.
*MaternalCareCoordination (Baby Love Program)The Baby Love Program can help you and your baby from the beginning by providing maternity care coordination, chidbirth education classes, counseling, and emotional support; referrals to other programs like WIC, dental care and family planning. Working on concerns that may effect the baby's health. Assistance in applying for Medicaid: No matter where you receive your medical care, you can sign up for the MCCProgram."Se habla Espanol".

Housing Services
* Transitional Housing with Supportive Services. Up to two years to stay and must meet special qualifications.
*Homelessness Prevention providing emergency assistance to keep families in their home such as rental assistance and utility bill assistance to prevent cutoff. Call for qualifications.
* Housing Counseling by a certified Counselor of The Association of Housing Counselors. First Time Homebuyers Classes. Access to special Loan Programs for low-inome individuals. Access to affordable housing and special housing programs and home buyer education material.
* HOPWA-Tenant Based Rental Assistance(HOPWA-TBRA).A special rental program for indidvuals with disabilities. Call for details or click here for application. Serving residents of Harnett, Johnston, Sampson and Dogwood Consortium Counties.

Immigrant Services
Providing assistance with the immigrant population with such things as: translation, Notary Services, access to BCIS documents & forms, assistance in completing BCIS Forms and assistance in accessing local social services. In addition, Citizenship Classess are offered on a quarterly basis. "Se habla Espanol".
- 21/2 hours in class 2 days a week for 40 hours.
- Books and material provided
- Completion of the N-400 ( application for Citizenship)
- Video & audio tools.
